When Is Back Surgery Actually Necessary?

Back surgery is sometimes necessary — but it is far less common than people think.

True surgical necessity usually involves:

  • Progressive neurological deficits
  • Severe spinal instability
  • Loss of bladder or bowel control
  • Fracture, tumor, or infection

What surgery does not do well is heal discs. Surgery removes tissue, stabilizes segments, or decompresses nerves — but it does not restore disc health.

Many patients are advised to pursue surgery because symptoms haven’t resolved quickly, not because surgery is the only option.

The key question should never be:

“Does my MRI look bad?”

It should be:

“Can my disc still heal conservatively?”

That’s a very different conversation.
